Documents You Must Gather (as Sponsor in Italy)
Proof of family relationship:
- Birth certificate of the adult child (original, translated into Italian, and legalized via apostille)
- Marriage certificate of the child's parents (if applicable, translated and legalized)
- Adoption documents (if applicable, translated and legalized)
Income documentation (choose the category that applies):
If you are a dependent employee:
- Latest tax return (CUD if single employer, or Mod. 730/Unico with electronic filing receipt)
- Last three months of pay slips
- Employment contract or hiring letter (UNILAV form)
- Model S3 form (employer declaration) with employer's ID copy
If you are self-employed or a business owner:
- Chamber of Commerce registration (not older than one month)
- Professional registration (if applicable)
- Municipal business license (if required)
- Company articles of incorporation
- If operating more than one year: Latest tax return (Mod. Unico) and accounting report prepared by an accountant
- If operating less than one year: Accounting report covering the entire operating period
Housing documentation (choose the category that applies):
If you own the property:
- Family status certificate from your municipality
- Housing suitability certificate from your municipality
- Copy of notarized property deed
- Model S2 form (if property is co-owned)
If you are renting:
- Family status certificate
- Housing suitability certificate
- Copy of registered rental contract (must be valid for at least 6 months from application date)
- Model S2 form (if co-rented)
If you are using a free loan (comodato gratuito):
- Family status certificate
- Declaration of other cohabitants
- Housing suitability certificate
- Copy of comodato contract (must be valid for at least 6 months)
- Model S2 form signed by the property owner
Residence permit documentation:
- Copy of your valid residence permit (permesso di soggiorno)
- Proof of at least two years of continuous legal residence (residence permit history, entry/exit stamps, or municipal registration records)
Application form:
- Completed application form (available on the Ministry of Interior portal)
Documents Your Adult Child Must Gather (Abroad)
Identity and travel:
- Valid passport (with at least two blank pages)
- Recent passport-sized photographs (2x2 inches, color, full face, front view)
Family relationship proof:
- Birth certificate (original, translated into Italian, and legalized via apostille)
- Marriage certificate of parents (if applicable, translated and legalized)
- Adoption documents (if applicable, translated and legalized)
Medical documentation (critical for this pathway):
- Medical certificate issued by a doctor appointed by the Italian consulate in the country of origin
- The certificate must explicitly document total disability (100% invalidity) and permanent inability to support oneself
- You (the applicant) bear the cost of this medical examination (typically €100–€300)
- Gather supporting medical records: hospital reports, specialist evaluations, disability assessments
Health insurance (if required by the consulate):
- Proof of private health insurance valid for the Schengen Area
Visa application form:
- Completed National Visa Application Form for Family Reasons
- Must be signed in the presence of a consular officer
Translation and Legalization
All documents issued abroad must be translated into Italian by a professional translator and legalized. Legalization is typically done via apostille (a simplified certification under the Hague Apostille Convention). If your country does not participate in the Hague Convention, documents must be legalized through the Italian diplomatic mission in your country.
